Q. I am having difficulty accessing this survey. Why?

A. Your Username/Password is your unique access code and can only be used once to complete the survey, so please do not share it with any one. If you are unable to gain access to the survey by entering your Username/Password, please ensure you are typing the correct combination of letters and numbers.

Q. How do I re-start my survey if I need to exit?

A. (1) To exit, close your browser by selecting the "X" in the upper-right corner of your window or select close from the file menu. (2) To re-start, go to the original address provided to you. (3) Re-enter your Survey Password. The survey program will take you back to your last page.

Q. I do not seem to be able to connect to the Internet, or I am being prompted to "Enter Network Password", or I am getting a "Proxy Server Failure". How do I correct this problem?

A. If you normally use a User Name and Password to gain access to the Internet, use the User Name and Password provided by your company. Use the Survey Password provided in your notification message to gain access to the survey.

Q. I received one of the following error messages: "Server Error", "Server Timed Out", or "Resources Exceeded". How do I correct this problem?

A. Use the Back button on your browser to take you back to your previous page. Then use the Next Page button at the bottom of the page to re-submit the page.

Q. Where do I send my survey after completion?

A. You do not have to send us your survey. The survey program saves your data every time you submit a page. Once you have reached the end, your survey is complete and all of your data has been saved. You should then close your browser to secure your data.
